How do you find me?
Location:
My office is located near the intersection of Douglas and Eureka . If you go South-east on Eureka, just past Douglas turn from the left hand turning lane onto Professional Drive. You then turn into the second driveway on the right side of the street. Go to the left into the parking lot, and then turn on the first turn to the left. My bulding is the second building on the left hand side, with the numbers 2330 by the sidewalk in front of the building. Go into suite 100 and you will be in the waiting room.
Payment:
Accept cash, checks, debit cards, VISA, Mastercard, American Express, Discovery and E-checks.
Insurance:
I can bill directly to MHN, if you get prior authorization.
All other insurance companies: If you have a PPO that allows you to choose your own therapist, I will prepare the forms for you, you send them in to the insurance compay, and the reimbursement will come directly to you.
